After the death of the God Aki, his mother, Larsa, begins to deteriorate mentally. The distraught woman manipulates her other 9-year-old son Palm into traveling through the dangerous Shinju Forest to retrieve the Koju Goddess Reco. Palm arrives and weaves a tale of deadly Koju beetles and other giant bugs attacking his village. Reco and Palm make their way through the dangerous Shinju Forest to save the village of Utakata to battle Larsa.

Mushihimesama Futari Ver 1.5 features different game modes: Original Mode, Maniac Mode, Ultra Mode, and the exclusive Arrange Mode. Each mode is unlocked at the start of the game and features increasing difficulty levels.

Considered a bullet hell video game, Mushihimesama Futari pits players against a barrage of enemy forces and enemy fire. In each stage, players will have to take on swarms of evil giant bugs and enemy fighters and tackle both a mid-level boss and a big boss. The true challenge in the bullet hell titles is figuring out bullet and enemy patterns to stay alive.
This offshoot of the general shooter gaming genre has remained popular long after its introduction. Fight your way through each stage shot by shot, collecting gems from defeated enemies to fill your counter. The player’s gem counter will change the color type from green to blue every 2,000 gems, requiring use of different weapons. As the counter switches colors, only a shot from the right weapon will count. After each stage, each counter will reset to zero. With updated version 1.5, many positive changes were made, such as modified bullet patterns, item changes, and a few game system and stage content changes. These changes were incorporated to make them accessible to novice players and hardcore gamers alike.
